Pet Hospital registration
To register your pet with your local PDSA Pet Hospital, you will need:
- Your contact information
- Your pet’s details
- Proof of benefits
- Your photo ID (driving license, passport, bus pass)
- A recent, clear photograph of yourself
Pet details
With this form you can register one dog, cat, or 'small furry' (for example, rabbit or hamster) with your local PDSA Pet Hospital. We can add additional pets to your account once
your application has been approved.
You will need to provide your pet's name, species, breed, colour, gender and age.
Proof of benefits
You should qualify for our FREE service if you are on any of the following benefits:
- Universal Credit (with housing element)
- Housing Benefit (means-tested help with your rent)
- Council Tax Support/Reduction scheme (means-tested help with your council tax liability)
Please note: You are not eligible for PDSA services if you have single occupancy council tax discount alone.
You should qualify for our LOW COST service if you are on any of the following benefits:
- Universal Credit (without housing element)
- Child Tax and Working Tax Credits
- Pension Credit
- Income Support
- Jobseeker's Allowance
- ESA - (Income Based Only) Employment Support Allowance
- Disability Living allowance (DLA) / Personal Independence Payment (PIP) / Adult Disability Payment (ADP)
- State retired pensioners who are householders in council tax bands A-D
You will need to provide a picture or scan of your latest benefit document(s) as proof of eligibility.
To upload, you can take a photo of your benefit document or have a file/image ready to upload at the next stage.
PDSA will delete your documents once your application has been processed.
Important details
Your benefit document(s) MUST clearly show:
- Name and address - the details on your benefit document(s) must match your application details
- Date of document issue - must be within the last 12 months (contribution related ESA must be dated in the last 6 months)
- Benefit amount (may be referred to as reduction, support, award or exemption)
If the documents you upload do not clearly show all of these details, there will be a delay in processing your application.
Recent photograph
You will need to provide a recent, clear, passport-style photograph of yourself in addition to your ID document for verification.
Your photograph must be of your head and shoulders and be taken against a plain background.
You can upload a photo of yourself from a smartphone.

We aim to process all applications within 2 working days.

- Your registration is not active until your application has been approved
- If we can't confirm your eligibility, we will contact you via email
